Terms and Conditions:
Refunds Due To Missed Classes - There are no refunds for missed camp, after school programs, or sports classes and no make up dates will be offered. I understand that my child’s attendance is my responsibility. EYS closes for all hurricane and tropical storm emergencies and no refunds will be given.
Payment Collection - All payments will be collected by Extreme Youth Sports Inc.’s billing system on a weekly basis for our summer camps and after school programs. Tuition is collected on a monthly basis for all sports classes. I understand that I am responsible for making the most of my purchase by attending all weeks of camp, after school program days, and sports classes, that I have registered my child for.
Summer Camp Tuition - If you purchase the entire summer with one payment, or the gold special, which is 10 payments that EYS financed at 0% interest, your purchase is non-refundable or transferable. If I purchased the gold special (10 payments) or I have purchased individual weeks, I understand that tuition is collected seven days prior to the weeks that I have purchased. All gold special purchases begin billing on 6/4/12 and continue every Monday until all 10 payments have been successfully collected. The method of payment I used to create my account will the used for tuition collection. If my card declines for any reason during the tuition collection process, I understand that the EYS system will automatically try to collect the charge up to 30 more times over the next 30 days. If the tuition is still not collected by Wednesday of the week before camp starts, EYS will contact me to use a different method of payment. If the payment is not available, I will lose my child’s spot to another person who is on the waiting list and my child will not be allowed to attend camp that week.
Summer Camp Cancellation Policy - If I decide to break my 10 week entire summer agreement/contract, I will forfeit the discount that I have already received and I will be responsible for paying the same rate of the weekly membership for all previous weeks that my child attended. This averages out to be $22 per week per child plus a $30 cancellation fee per child.
If I decide to cancel any single purchase weeks, I must contact Extreme Youth Sports in writing no later than 7 days before the next billing cycle or I will be charged for the following week – No exceptions. All cancellations must be made in writing by sending an email to extremeyouthsports@yahoo.com. I also understand that it is recommended that if I cancel via email, I should still follow up with a phone call to (813) 817-KICK to confirm receipt of the email. I understand that all full summer, one time payments, and the gold special which is 10 payments, are not refundable or transferable to others and it is my responsibility that my child attends.
Rescheduling weeks of Camp - If I purchased individual weeks and I have to reschedule a week of camp, I will be charged a $30 administrative fee. All rescheduling must be done within 7 days of the start of the week of camp, or I will be responsible for paying full price for the week I missed. One time payment memberships and gold (10 week – full summer) memberships can not be canceled or rescheduled.
After School Program Tuition - There are two tuition plans for our after school program. The first one is considered the “Monthly Membership” and it’s tuition is $110 per week per child. You may cancel anytime with a 30 day notice without future penalties or payments.
The second plan is considered the “Entire School Year” membership and it’s tuition is $57 per week per child. This plan costs a total of $2,394 and there are 42 consecutive weekly installments of $57 financed at 0% interest. This plan is a contract and can not be cancelled.
After School Program Cancellation - If you break your “Entire School Year Membership” contract early, an early termination fee of 30% of the remaining balance will be applied to your account for each child, regardless of the number of payments you have already made.
Missed Payments - If I default on any of my 42 payments of $57, I will be contacted for a new method of payment. If I am unable to provide a new method of payment, my child will not be allowed to attend the After School Program and my account will be cancelled if arrangements are not made to make future payments. The early termination fee will be applied for breaking my after school program contract.
After School Program Closings - I understand that early release days are included in my tuition, however the following closings will require an additional fee: All n0n-holiday single day school closures, Thanksgiving break, Winter Break, and Spring break camps. As an after school program member, I will receive a discounted fee as well as the first opportunity to register compared to the public. *I understand that if I purchased the $2,394 Entire School Year membership, that the 42 weekly payments will start 7 days prior to school starting and end during the last week of school. I also understand that my $57 weekly payments will still be due over the Thanksgiving break, Winter Break, and Spring Break holidays even if my child does not pay extra to attend the camps during those weeks because camps are not affiliated with the after school program.
Weekly Sports Class Tuition - I understand that all sports class tuition is collected monthly on the first of each month. If I decide to cancel my child(ren)’s classes, I must provide a 15 day notice by clicking on the cancellation link and filling out the cancellation form located on our website www.ExtremeYouthSports.com
Karate Belt Testing - My child needs to attend a minimum of 8 classes to be eligible to test for a stripe on his/her belt. Once 3-4 stripes have been earned, my child will receive an invitation to test for his/her next belt color. If my child passes, he/she will be rewarded a new color belt. If he/she fails the test, he/she can retest during the next testing, one month later. All black brown belt students must obtain that belt for one full year before being invited to test for their first degree black belt.
Late Fees - I will be charged a late fee of $1 per minute after I am 5 minutes late picking up my child, that must be paid before my child is allowed to return to the summer camp or after school program the next day.
Pick up Policy - If someone other than a parent or legal guardian is picking up your child, EYS must be informed the day before and that person must present a valid photo ID in order for us to release your child to them.
Snacks - I understand that my child is responsible for his/her own meals and snacks during summer camp and after school programs. Your child will need to bring a bagged lunch, drinks, and two snacks each day to eat for summer camp and one snack each day for our after school program.
Field Trips - We will be going on a few field trips during the summer. Field trip fees are not included in your child’s weekly tuition and you will be charged extra if you allow your child to participate. We will inform you 2-3 weeks prior to each field trip with an email. You can enroll by completing an online registration form on our website.
Registration Fee - Upon registering for summer camp, after school program, or any of the weekly sports programs, I understand that I am paying a one time deposit (registration fee) of which secures my child’s spot. This payment is non-refundable and it reserves my child’s spot in the program(s) I have chosen. If I decide not to attend any of these programs after I pay my registration fee, I will forfeit this deposit. The method of payment I used to pay my registration fee will be used to establish my account with Extreme Youth Sports Inc. and will be used to pay tuition, unless I contact EYS to change the method of payment. I understand that EYS does not accept cash or checks for tuition payments.
Marketing - I give Extreme Youth Sports permission to use photos and video footage of my child during class for promotional purposes.
Liability Waiver – By registering my child in any Extreme Youth Sports Inc. program or activity, I hereby agree to let my child participate in the sport or activity selected. I understand that there are certain risks of injury inherent in the practice and play of the sports activities that are offered by Extreme Youth Sports Inc., as well as in traveling and other related activities incidental to my participation, and am willing to assume these risks. I hereby certify that my child is fully capable of participating in this activity and that he/she is healthy and has no physical or mental disabilities or infirmities that would restrict full participation in this activity, except as included in writing with this application. In addition to giving full consent for my child’s participation, I do hereby waive, release, hold harmless, and covenant not to sue Extreme Youth Sports Inc., its officers, coaches, sponsors, supervisors, and representatives for any injury that may be suffered by my child in the normal course of participation in this activity and the activities incidental thereto, whether the result of negligence or any other cause.
By enrolling in the EYS summer camp, EYS after school program, or any of the EYS sports classes, you must acknowledge and agree to these policies and requests by clicking the box stating I have read and accept these terms and conditions, on our online registration form. By enrolling your child and accepting our terms and conditions, you are giving an electronic signature which is the equivalent to a real hand written signature.
